Release of Liability:

I, the submitter of this form, hereby acknowledge that I voluntarily wish to participate in the above-mentioned event (the “Event”) organized by Ascend Beyond Pediatric and Family Therapy. I understand that the Event may involve certain risks, including but not limited to tripping and falling, injury, choking hazards, etc. and I freely and voluntarily assume these risks.

In consideration for being allowed to participate in the Event, I, for myself, my heirs, executors, administrators, and assigns, hereby release and discharge Ascend Beyond Pediatric and Family Therapy, its officers, directors, employees, agents, volunteers, sponsors, vendors, and affiliates (the “Released Parties”) from any and all liability, claims, demands, actions, or causes of action that may arise due to my participation in the Event, including, but not limited to, personal injury, property damage, or death, whether caused by the negligence of the Released Parties or otherwise.

Indemnification:

By submitting this form, I agree to indemnify, defend, and hold harmless the Released Parties from any and all claims, damages, costs, and expenses (including legal fees) that may arise as a result of my actions during the Event or any breach of this waiver and release.

Medical Treatment:

By submitting this form, In the event that I require medical treatment during the Event, I hereby consent to the administration of any medical care deemed necessary by a qualified healthcare professional. I agree to be responsible for any medical costs incurred.

Photographic and Media Release:

By submitting this form I grant Ascend Beyond Pediatric and Family Therapy and the participating companies (venue, photographer, partnering programs) the right to use any photographs, video recordings, or other media taken of me during the Event for strictly promotional purposes, including but not limited to use on websites, social media, and other marketing materials, without compensation.

Acknowledgment of Understanding:

By clicking submit for this form, I acknowledge that I have read and fully understand this waiver and release form. I understand the risks involved in participating in the Event and voluntarily assume those risks. I am aware that by clicking “Submit,” I am signing this form, I am waiving certain legal rights, including the right to sue the Released Parties.

If Participant Is Under 18 (Parental Consent):

I, the submitter of this form, am the parent or legal guardian of the minor participant named above. I have read and understood the above waiver and release form, and I consent to the minor’s participation in the Event. I further agree to indemnify, defend, and hold harmless the Released Parties from any claims that may arise out of the minor’s participation.

By submitting this form, I agree that this Release Form will be valid for myself, my guest, and my family’s attendance at future Ascend Beyond Pediatric and Family Therapy events for the duration of 3 to 6 months of submission of this document.
